Emmerdale could be set to welcome back a notorious villain in a shocking Dr Todd plot twist.

Dr Todd (Caroline Harker) has been making medical student Jacob Sugden's (Joe-Warren Plant) life an absolute misery in recent weeks. Not only has she been pushing him beyond his limits, but Dr Todd has also been gaslighting him and repeatedly calling him out over his errors.

Last week, however, it emerged that Dr Todd's dementia-suffering father had been hospitalised and was in a serious condition. Dr Todd and Jacob appeared to share a touching moment of connection, with the doctor also revealing she had a wife at home, offering a rare glimpse into her personal life.

Yet in Tuesday's episode (April 7), events took a dramatic turn when Dr Todd's father suffered a cardiac arrest. With nobody else present, Jacob stepped in to perform CPR on him — despite the patient being marked as DNR (Do not resuscitate).

When Dr Todd discovered what had happened, she was furious, confronting Jacob over his decision to perform CPR on her father. She even told him he lacked the qualities needed to become a doctor, before sending him home, reports the Daily Star.

Now, however, viewers believe a sensational return twist is on the cards, with many predicting that Dr Todd's sister is none other than Maya Stepney (Louisa Clein).

Fans will remember that Maya groomed Jacob when he was a teenager, sleeping with him shortly after his 16th birthday. Maya's predatory actions ultimately led to her arrest and imprisonment, though she was released early from gaol in 2019.

"Dr Todd playing mind games with Jacob, still reckon Mad Maya is her sister hence revenge," speculated one viewer on an Emmerdale Facebook fan page. Another suggested: "Maya's sister here for revenge." While a third concurred: "Bet she's Maya's sister."

Meanwhile, discussing Dr Todd's comeback, Caroline Harker said: "I'm delighted to be back at Emmerdale for a longer stint playing Doctor Todd.

"She's a complex character and from what I've been told so far, quite a tricky one for Jacob to navigate." The accomplished actress went on to say: "Workplace bullying is commonplace, and it will be interesting to see it brought to our screens in a serial drama such as Emmerdale."
