The 79-year-old took to the stage with Faye Dunnaway to reveal who had triumphed in the prestigious category but was left red-faced when he handed the gong to La La Land instead of Moonlight.
The cast and crew of the film fronted by Emma Stone and Ryan Gosling rushed onto the stage to collect the statuette.
But, in the middle of their thank you speeches, they were interrupted and told there had been a disastrous mix-up.
One of the film's producers, Jordan Horowitz, explained: "No there has been a mistake. Moonlight, you guys won Best Picture."
Warren was left embarrassed by the error, with the card revealing Moonlight had won being ripped out of his hands and shown to the audience.
