The 23-year-old joined the Teeside club in a four-and-a-half-year deal for an undisclosed fee.
The striker leaves Stamford Bridge after failing to make a senior appearance since arriving in 2012.
He had six loan spells away but his best season came in 2014-15 when he scored 19 goals for Middlesbrough.
But Bamford has expressed his appreciation to Chelsea in a heartfelt message on Twitter.
He said: "I would also like to thank Chelsea for the past five years, and all the staff and players at the club for the constant help and support they gave to me.
