Downing drops down from the Premier League to rejoin his home-town club, who spent a reported £5.5million to land the England international.
The 30-year-old, who played almost 250 games for Boro, left the Riverside for Aston Villa in 2009 before moving to Liverpool.
Downing failed to live up to his £20m price-tag at Anfield and was sold two years later to West Ham for just £5m.
The England international will now link up with Aitor Karanka's squad at a pre-season training camp in Marbella.
A Boro statement read: "The club is delighted to confirm the signing of Stewart Downing from West Ham United for an undisclosed fee.
"Downing, a product of the Boro Academy, completed his move on Thursday evening, signing a four-year contract, and is flying out to link up with Aitor Karanka’s squad in Marbella."
Downing said on the deal: “I always said I wanted to come back to this club and I’m delighted the chance has now come. I didn’t put a time limit on it but now the circumstances are right.
“There was a shout about me coming back here on loan when I was at Liverpool, but for one reason or another it didn’t happen. But now I’m here, and I’m absolutely buzzing."
