Tottenham sit fourth in the Premier League table, five points ahead of fifth-place Chelsea.
The two sides clash at Stamford Bridge on Sunday (4pm) in what is likely to be a must-win match for Antonio Conte.
Conte could still leave Chelsea even if they do secure Champions League football next season and win the FA Cup.
And the Telegraph claim Argentine Pochettino is on Blues owner Roman Abramovich’s managerial radar.
According to the report, Chelsea are aware of the difficulties prising Pochettino away from Tottenham would involve.
Spurs have developed well since Pochettino joined from Southampton and are now a steady top-six club.
The Telegraph claim Conte’s exit is likely and that Chelsea will make a move for Pochettino if Spurs fail to reach the Champions League.

Pochettino has also been linked with clubs abroad in recent weeks, with Real Madrid, Barcelona and PSG all supposedly interested.
The 46-year-old has ruled out ever taking charge of Barca but, with Zinedine Zidane and Unai Emery both under pressure at their respective clubs, a move to Madrid or Paris is certainly a possibility.
