According to Don Balon, Pep Guardiola has been in touch with the Chelsea forward.
And the Spanish publication claim Hazard is open to making a move to the Etihad Stadium.
Guardiola has reportedly decided to go for Hazard after cooling his interest in Alexis Sanchez in January.
The Belgium international is also a target for Real Madrid.
Express Sport exclusively revealed last month that the Spanish giants were preparing to offer £100million plus Gareth Bale for Hazard.
Chelsea hope to extend Hazard’s contract to scare off his suitors but talks stalled earlier in the season.
The Blues have him tied down until the summer of 2020 but he previously claimed he would only put pen to paper when Thibaut Courtois commits his future.

Chelsea were attempting to secure Courtois’ signature in January but the Belgian goalkeeper is yet to sign on the dotted line.
He is also attracting interest from Real Madrid and hopes to resolve his future before the World Cup.
Despite previously suggesting his future is intertwined with Courtois, Don Balon claim Hazard has decided he wants to leave Chelsea.
A move to City would be controversial but he will reportedly consider any potential destination.

Chelsea are struggling to extend Eden Hazard's contract
Chelsea could lose the likes of Hazard if they miss out on Champions League football next season.
But head coach Antonio Conte, who is unlikely to remain in charge beyond the end of this campaign, does not believe missing out on a top-four spot will affect the club’s chances of keeping hold of their star men.
"Don’t forget two years ago Chelsea finished 10th with many players of this season...they decided to continue to play for this club," he said.
"Many players they were here when we finished 10th. This is not a question for me, it is a question for the players."
