The Bosnian has had an excellent season for the Serie A side, scoring 12 goals in 43 games.
The 26-year-old has contract until 2018 with a reported buy-out clause of over £30million.
But according to the Evening Standard, Roma fear Pjanic has already made a decision to leave the club.
And Chelsea are ready to offer him a three-year-deal worth over £65,000-a-week.
Incoming Antonio Conte is understood to be a fan of Pjanic and is already plotting his summer transfer targets.
Pjanic's team-mates Radja Nainggolan and Kostas Manolas are also on Chelsea's radar.
