Hazard, 24, is understood to be disappointed with life in west London after being dropped for big games this season.
He has been benched for three of Chelsea’s last seven games and is yet to score for his club this season.
Hazard joined Chelsea from Lille in a £32m deal back in June 2012 and signed a new five-year contract eight months ago.
He was instrumental in the side’s charge to the Premier League title last season but this term has not hit similar form.
It is not known if he will play for the Blues at Stoke in Tuesday’s League Cup clash, with home ties against Liverpool and Dynamo Kiev to follow.
