Philippe Coutinho's early opener for the Reds was cancelled out at Anfield deep into the second-half through QPR midfielder Leroy Fer.
The hosts had the chance to regain the lead five minutes later when Nedum Onuoha's shirt-pull on Martin Skrtel conceded a penalty.
Last time I changed my mind I missed as well
However, Gerrard failed to convert from the spot because he second-guessed himself in the run-up.
He said on the miss: "I changed my mind on the run-up and rule number one with penalties is you don't change your mind. Last time I changed my mind I missed as well."
The former England midfielder made up for his mistake to score the winner moments later when he headed home from a Coutinho corner.

On the goal, he added: "I was happy with my header, it wasn't the best I have scored, that was my header in the Champions League final.
"It was a good three points, we have to stay strong till the end and keep pressure on the teams above us."
