It is believed the chopper was showing people around Hawaii when it hurtled towards the tranquil water.
The 16-year-old boy was rushed to hospital and is now in a critical condition with two others, a 50-year-old man, and a 45-year-old woman in a stable condition.
The other two passengers are not believed to be critically injured.
Horrified onlooker Max Burner said he saw smoke from the stricken Bell-206B chopper just before hitting the water and claims he saw the helicopter “sputtering” around 500ft before impact.
He said: “As soon as it hit the water, some people popped out”.
Mr Burner jumped into the water to help free passengers with other witnesses cutting people free from a seatbelt in the chopper.
It was not spinning out of control, but it was dropping at a good rate
Another witness, Roy Gano said the aircraft “hovered for a second and then started to descend”.
He said: “It was not spinning out of control, but it was dropping at a good rate. It narrowly missed the rocky edge and turned to its side. I saw people dive into the water to help.”
