What is left to be decided?
There are three issues to be resolved - who will qualify for the Champions League, who will go down and who will win the Golden Boot.
Liverpool may have enjoyed a stunning season, propelled by Mohamed Salah, but they still have not secured a Champions League spot for 2018-19.
They sit fourth going into the final day with a two-point advantage over Chelsea. A superior goal difference means they need a point from their home game with Brighton to secure fourth.
Chelsea, meanwhile, face Newcastle. They need to win and hope Liverpool lose to seal an unlikely Champions League finish.
At the bottom end of the table, Stoke and West Brom are already down.
It's likely that Swansea will join them but that is still to be confirmed. The Swans are three points behind Southampton and a goal difference which is nine worse off.
The only chance they have of surviving is to beat already-relegated Stoke by a big margin.
They then need to hope Manchester City thrash Southampton to ensure a huge goal difference swing that they need.
