Bale looks set to be dumped by Real Madrid at the end of the season as part of club president Florentino Perez’s summer revamp.
According to Don Balon, the Wales international accepts that he is on borrowed time at the Bernabeu.
The Spanish publication claim Bale is ’crazy’ about a return to the Premier League and is attracting interest from Chelsea.
Express Sport understands Real Madrid will offer Bale plus £100million to the Blues for Hazard.
And, according to Don Balon, Stamford Bridge chiefs are open to a swap deal involving the two players.
Bale, who moved to Spain from Tottenham in 2013, reportedly welcomes the idea of playing for Chelsea.

The 28-year-old has scored 77 goals in 175 appearances and won 10 trophies since his then world record move in 2013.
But an injury-hit past 18 months and the rise of Marco Asensio has raised speculation over his future.
Bale has struggled to win a place in head coach Zinedine Zidane’s starting XI since returning from his latest spell on the sidelines.
Real Madrid want to sign Eden Hazard from Chelsea
He was a substitute for the crunch Champions League clash against Paris Saint-Germain last month but Zidane insists he is carefully managing the forward back to fitness.
"I am happy with him," the Frenchman said. "We have to go bit by bit with him.
"What we want is for him to hit his best and we're getting there.
"We'll look at the matches that are coming up next and at how we're going to manage them.
"We are good and in good form and I am counting on all my players.”
Eden Hazard takes on Gareth Bale
Hazard is stalling on signing a lucrative new contract at Chelsea amid interest from Real Madrid.
His current deal expires in 2019 but, speaking last November, he said: “Everyone knows the admiration I have for Real, but for now I'm a Blue.”
Reports in Spain suggest Hazard will only consider extending his Chelsea contract if a release clause is included.
That would make it easier for Real Madrid to lure him to the Bernabeu.
