Police in Austria said the attacker was overheard having a 'loud argument' with a woman shortly before carnage at an open-air venue near the town of Nenzing, in Vorarlberg, western Austria, at about 3am Austrian time, 1am in Britain.
The 27-year-old gunman, who has not been named, walked to his car in a nearby car-park and pulled out a gun before shooting at a crowd of 150 concert-goers.
Two were killed instantly in the attack
Eleven more were injured by the gunman
He then returned to the car park and killed himself, Austrian state broadcaster ORF said.
The woman he had the row with, believed to be his girlfriend, is not reported to have been injured.
ORF says the shooting occurred on property used by motorcycle club The Lords and a "stampede" broke out as people desperately tried to run from the shooter.
Two concert-goers, a 33-year-old man and a 48-year-old man, both from the local area, were killed instantly.
