The 47-year-old former footballer was pictured leaving a shop in Poole last week wearing little more than a coat and jeans, carrying a shopping bag.
It's thought his shopping included a green glass bottle of what appeared to be alcohol, but this is unconfirmed.
The pictures were thought to be taken just one week after his successful appearance on BT Sport where he looked fit, healthy and on the road to recovery.
Many football fans have taken to social media to question whether Paul, fondly known as Gazza, needs help for alcoholism again.

Speaking back in November, after a successful stay in rehab, Paul opened up to This Morning presenters Eamonn Holmes and Ruth Langsford about his most recent relapse.
He said: "This time, I knew I was in trouble straight away so I tried to put a block on it, and I was just detoxing in my house. Then all of a sudden, I just felt I was set up by a few people, and I carried on drinking."
And just last week Gazza's former team mate Gary Lineker spoke out in support of the star.

Speaking exclusively to the Daily Express Online, 54-year-old Gary said: "I wish him all the best. He's had a tough time, hopefully he can get his health back on track and he can have some structure and stability in his life.
"I just really wish him well. He's got some documentary film on his life and I'm doing a little bit for that. Hopefully, hopefully he'll stay on the up."
He added: "I think everyone needs a bit of support in life - especially our friends and people we've been involved with. If you can help it's great."
