The 49-year-old Hollywood star beat the likes of Jennifer Aniston, 48, and Michelle Pfeiffer, 58, who have both held the title twice.
Julia was first awarded the honour back in 1991 at the age of just 23 and her record fifth win means that she flies ahead of her Money Monster and Ocean's franchise co-star, George Clooney, 55, who has won Sexiest Man Alice just twice.
She joked that she would be teasing her A-list pal about her victory, telling the magazine: "I'm going to mention that in my Christmas card to the Clooneys this year."
Julia first won the accolade back in 1991
Julia, who first shot to fame when she starred alongside Richard Gere, 67, in the beloved 1990 movie Pretty Woman, said she was "very flattered" to be named Most Beautiful Woman yet again, adding that she feels that the accolade comes at the height of her professional and personal life.
"I think I'm currently peaking," she said.
Gracing the cover of the World's Most Beautiful issue, Julia beamed her trademark stunning smile, wearing a grey knitted jumper and soft smokey eye make-up adding a dash of glamour with fuchsia pink lipstick.
With her coppery brunette locks styled into gentle waves that cascaded loose over her shoulders, the actress looked impossibly youthful as she posed for her victorious cover shoot.

Julia gushed about her husband Danny Moder
The mother-of-three also opened up about her 14-year marriage to husband Danny Moder, gushing about how grateful she was for her loving relationship, saying: "Every day my husband walks in the door it's like a recurring dream. I'm like, 'Ah, he's back!'"
She also admitted that she wasn't aware how much her iconic breakthrough appearance in Pretty Woman was going to launch her career.
"I remember reading: 'Pretty Woman came out this weekend and made this much money', she recalled, explaining that she was out of town the day the movie was released. "And I thought, is that a lot of money? Is that good? Is that great? I really don't know."
People's 28th annual World's Most Beautiful double issue will hit newsstands on Friday.
