The 56-year-old was snapped leaving rehearsals for tonight's hotly-anticipated performance at London's 02 arena, last night.
Surrounded by her dancers, and members of her personal team, the star was grasping an inhaler.
Holding it near her face, Madonna looked tired as she made her way down an escalator.
Wearing all-black, including a black hoody, the star was dressed down with her blonde locks plaited into a comfy style.

Her turn on stage this evening - opening the show - will mark 20 years since her last performance when, in 1995, she belted out her top 5 single Bedtime Stories for fans.
This year, the star will give the audience a show-stopping preview of her thirteenth studio album - Rebel Heart - set to be released in March.
Clearly in the middle of heavy preparations for her big opening performance, it looked like Madonna treated her dancers to a hot meal the night before the show.
